Additional Information

A formulary booklet includes a list of prescribed medications or other pharmacy care products, services or supplies chosen for their safety, cost, and effectiveness. Medications are listed by categories or classes and are placed into cost levels known as tiers. It includes both brand and generic prescription medications. Where differences between this formulary and your benefit plan exist, the benefit plan documents rule. This may not be a complete list of medications that are covered by your plan. Please review your benefit plan for full details. Medications may change on this list, move to a different tier or be excluded from coverage on January 1 or July 1 of each year.

Optum Rx HDHP Preventive Drugs List includes medications for which deductible will not apply under plans that include a deductible (e.g. HSA plans). This means you’ll pay your copayment/coinsurance for these medications, whether you have met the deductible or not. This list is should be used as a reference and may not include all medications. The list also may change every year.
